How To Make a Giveaway Entry Form with Google Docs


I am a big fan of Google Docs forms. They are really easy to make and are a great way to collect information. The information collected goes directly into a spreadsheet. Gotta love that!
I use Google Docs forms to make giveaway entry forms all the time. I will show you how to make them.

To get started log in to Google Docs and select create: form from the drop down menu.

Name your form then add questions.
You can include as many questions as you want and can change the question type (text, paragraph text, multiple choice, check boxes, grid, scale or choose from a list). Each question answered will show up as a new column on the spreadsheet.

I always have my form include name and email address, to make sure they are filled out I click the box at them bottom that says make this a required question.

When you are finished with each question click the done box at the bottom. To edit a question click the pencil button on the right. To add a question or duplicate a question click the button to the right of the pencil button.

When your form is complete click save in the top right corner and click the more actions drop down menu. Click embed.
A new window will pop up with the html code for your form. Highlight the code and copy.
On your blog post click html on the upper left hand corner. Paste the html code for your Google Docs form. Switch back to compose mode on your blog and continue writing your blog post as usual. 

When you are finished it should look something like this.
